What Information Do I Need To Charter An Air Ambulance Flight?

A Patient And Passenger Preflight Checklist

Your Medical Air Transport Flight Coordinator will need to know the information listed below in order to properly arrange your itinerary and air ambulance flight schedule.

Please Note: While it would be very helpful for you to have this information ready when you call, you don't need to have all the information right on hand. So don't worry, we can get the remainder from the patient’s physician – no problem.

Information needed regarding the patient:

  1. Full Name
  2. Date of Birth
  3. Gender
  4. Weight
  5. Medications being taken, if any
  6. Primary Diagnosis (Why is the patient being transported?)
  7. Does the patient require an I.V.?
  8. Does the patient require a Foley catheter?
  9. Current vital signs: blood pressure, heart rate, respiratory rate
  10. Current mental status
  11. Past medical history
  12. Current doctor’s name and contact information
  13. Receiving doctor’s name and contact information
  14. Current (pick-up) location
  15. Destination hospital

Family riders or other ride-along passengers:

  1. Full Name
  2. Date of Birth
  3. Gender
  4. Weight

General:

  1. Desired departure time
  2. Weight and size of luggage
  3. Payment details
